Declaration Of Explicit Consent

With the scope of and in accordance with provisions on law no. 6698 Personal Data Protection Law (PDPL) and Clarification Text that has submitted by data controller Company, I agree that Companies ,as data controller, or Data Processors that have authorized by data controller Company with necessary precautions;

I have informed by the Company that within the frame of and Companies’ administration, management and financial operations, personal data can be processed within the reasons that stated with as it is required; under the PDPL article 5/2(a) it is expressly provided for by the laws, under the PDPL article 5/2(c) it is directly related to the establishment or performance of the contract, under the PDPL article 5/2(d) personal data has been made public by the data subject, under the PDPL article 5/2(e) data processing is necessary fort he establishment, exercise or protection of any right or under the PDPL article 5/2(f) processing of data is necessary for the legitimate interest pursued by the data controller.

I have informed by Company about the subjects that are stated below and gave explicit consent to my personal data to be processed and and in accordance with purposes provided in Clarification Text may be processed in compliance with general principles especially the principle of being stored for the period laid down by relevant legislation or the period required for the purpose for which the personal data are processed, stated in article 4 of PDPL, may be collected, recorded, stored in physical or electronic environment for proportionated time to the purposes for which they are processed, protected, altered, adapted, disclosed in according to law and transferred in country or abroad, retrievaled, categorized, may be processed on the internet or social media or may be prevented the use thereof.

I gave explicit consent to my personal data to be transferred by Company in accordance with purposes stated in Clarification Text to employees of the Company, group company (Company and/or it’s business associates, shareholders), legally capable state institutions and organizations, independent auditing firm pursuant to legal obligations and limits and proportionate to it’s actions, Company’s business associates and service providers in order to actions and services that are provided to fulfill.

According to PDPL article 11 and applicaple law, I acknowledge my right to request to the data controller, to learn whether personal data that belonged are processed or not, to demand for information as to if personal data have been processed, to learn the purpose of the processing of personal data and whether these personal data are used in compliance with the purpose, to know the third parties to whom his personal data are transferred in country or abroad, to request the rectification of the incomplete or inaccurate data, if any, to request the erasure or destruction of personal data, to request reporting of the operations carried out to third parties to whom belonged personal data have been transferred. I acknowledge the right to submit to Companies with information relating my identification, my request including explanations relating the right with filling the application form on Companys’ website through Registered Mail Adress (KEP), secure electronic signature, mobile signature, apply in written form or via electronic mail adres that has submitted and recorded or
through a notary public or other methods that stated in law.

I undertake that the personal datas that are shared are accurate and kept up to date and any changes in the information given shall be notified to Company .

I gave explicit consent to personal datas that are defined in PDPL to be used and transferred within purpose of processing, stored for the period needed and I informed, read and understood this document and Clarification Text.
